All entries must include the following information to be considered. Have everything ready before submitting.
- Internet Link or MP3s of at least two (2) original songs written and performed by your current band.
- Photo of your band and a brief bio.
- Download and be ready to fill out the Waiver Agreement and Financial Statement(s)

A completed Waiver Agreement must be signed by a parent or legal guardian for each band member under the age of 18 at time of registration. If all band members are over 18 years of age at time of registration, the Waiver Agreement must be signed by the Primary Band Representative listed above.
Financial Statement - Since this award is donated by a charitable trust, the IRS stipulates that the award must be made to an individual whose financial status meets the qualifications as established by HUD. Only one member of the band has to qualify under HUD income guidelines. The qualifying income levels are more generous than you might think, See Examples.
